RECEIVING QHC INFORMATION VIA SNAIL MAIL

--In order for you to receive a registration booklet, we need to have an accurate snail mail address from you. If you did not need receive a registration booklet this past year, but have received one in the past, then you need to advise us of that situation so that we can check the mailing data base.

PLEASE NOTE:

--First class mailing costs $2.00 which should be sent to us ASAP. ALL other booklets are sent bulk mail. Please do not send Self-Adressed, Stamped Envelopes.

--Due to size of booklet/data, this information cannot be e-mailed, but will be posted on the website.

--All out of country attendees must request the receipt of a registration booklet each year.
