For a Team/Group entry, the person who signs the entry form should be the person who will be handling the shipping information and actual shipping. Any prize checks won by an entry will be issued to that individual as well. Where quilt has more than one owner, distribution of any prize money is the responsibility of that individual. Please list group name or members of group that should be credited in the "Quilt Maker's Name(s)" text area. Please review the form and make certain data is complete. Incomplete forms will not be accepted. (This entry form may be copied.)

Exact measurement accuracy is critical to proper display. DO NOT GUESS! Make sure your name, address, quilt name and orientation (“top”) are indicated on the images. Mail this form with your printed images and entry fee ($10 per entry) to: QHC Entry, PO Box 503, Carlinville, IL 62626.

Approximate Value:
If your quilt is valued over $2000, you must submit documentation to that effect with your entry.
